IMPACT OF A PHARMACOTHERAPEUTIC FOLLOW-UP SERVICE ON CLINICAL OUTCOMES AND QUALITY OF LIFE OF ELDERLY PEOPLE RESIDENT IN RURAL AREAS

Pharmacotherapeutic monitoring is essential in the care of elderly patients, who generally have more than one chronic health condition and use polypharmacy. Thus, the study aimed to evaluate the impact of a pharmacotherapeutic monitoring service on the clinical outcomes and quality of life of elderly people living in rural areas of the study region. The study is characterized as a before-after intervention, longitudinal, prospective, carried out from September 2022 to June 2023. The sample was selected by convenience, consisting of individuals aged 60 years or over. The sociodemographic, anthropometric, clinical and pharmacotherapeutic profile was outlined, in addition to identifying and preventing negative results associated with medication (RNM) and measuring clinical outcomes and quality of life before and after carrying out the interventions. 32 elderly people were evaluated, with a prevalence of females, aged over 75 years (31.3%). As for anthropometric measurements and systemic blood pressure, 50% were above reference values. Of the RNM detected, the majority were related to necessity and safety. After pharmaceutical interventions, 64.1% of NMR were resolved, there was a significant reduction in SBP (p=0.01), DBP (p=0.02) and capillary blood glucose (p=0.03). Regarding quality of life, all domains showed improvement, with limitations due to physical aspects showing a statistically significant variation (p=0.04). The study proved to be favorable to optimizing the clinical conditions and quality of life of the participants, being an effective clinical service for continuous care for the elderly.
